Failure Mode and Effects Analysis (FMEA)

The best time to solve a problem is before it becomes a problem.

Overview

Every organization strives to deliver consistent quality, ensure operational reliability, minimize costs, and meet customer expectations. Yet many process failures, defects, delays, safety incidents, and service disruptions are not random; they are often predictable and preventable. Waiting until problems occur can result in rework, wasted resources, customer dissatisfaction, and lost business opportunities. To build more resilient and reliable processes, organizations need a structured, proactive approach to identifying and preventing potential failures before they happen. This is where Failure Mode and Effects Analysis (FMEA) becomes an essential risk management and continuous improvement tool.

This highly interactive workshop provides participants with a practical, step-by-step approach to conducting an FMEA. Through guided discussions, hands-on workshops, and real-world applications, participants will learn how to define the scope of an analysis, identify potential failure modes and their causes, assess risks, prioritize improvement opportunities, and develop effective risk reduction actions. By the end of the program, participants will have completed an FMEA for an actual workplace process and gained the confidence to apply the methodology to strengthen process reliability, improve quality, and reduce operational risks.

Learning Objectives

At the end of this program, participants will:

  • Explain FMEA’s purpose, principles, and benefits as a proactive risk management tool;
  • Define FMEA scope by selecting an appropriate process and mapping its key steps;
  • Identify potential failure modes, their effects, and root causes using a structured FMEA approach;
  • Assess process risks by applying Severity, Occurrence, and Detection ratings;
  • Prioritize improvement opportunities based on risk level; and
  • Develop risk reduction actions and complete the FMEA worksheet for continuous improvement.
